How can I take attendance using MS Forms?

Answer

Create a Contactless Attendance Sheet with Microsoft Forms

Rather than passing around paper sign-in sheets to take attendance, digital, contactless attendance can save valuable class time and improve the workflow and accuracy of attendance.

Microsoft Forms offers a simple and free way to set up contactless attendance in your class. Students will simply scan a QR code from their phone (or use a weblink) and answer a few simple questions to sign in for your class. The following guide will walk you through how to get started.

Creating an Attendance Form

  1. Sign into Microsoft Forms with your GSA Credentials
    • Alternatively, you can log into Office.com, look for the Microsoft 365 icons on the left, and then select Forms
  2. Select + New Form
  3. Title your form with the name and date of the class, and add an optional description if you wish
  4. Select +Add new to add a new question to your form
  5. For a simple attendance form, use a Choice question-type, then type a brief attendance question into the field that appears: such as 'Are you attending class today?' Yes/No
  6. Be sure to tick 'Required'
  7. If you wish to add additional questions, for example using this form to ask for student response to a prompt in the lesson, click +Add new

  8. Click 'Settings' (the three dots in the top right corner) and set as follows:

    • Enable Only people in my organization can respond. This will ensure that students need to log in
    • Record Name
    • One response per person
    • Set your start and end date and time to be limited to the class time
    • Include a thank you message if you wish
    • Add a graphic background for the theme if you wish
    • Preview your form on mobile phone view to see what students will view
  9. Once you are finished creating questions, click Share
  10. Select 'Only people in my organisation can respond'
  11. Select the QR code option, then select Download to save a copy of the QR code to your device
  12. You may print out the QR code for use in your classroom, or you can add the QR code to your PowerPoint slide deck
  13. If you want to use the link rather than a QR code, you can shorten it via bit.ly.com
