- Select +Add new to add a new question to your form
- For a simple attendance form, use a Choice question-type, then type a brief attendance question into the field that appears: such as 'Are you attending class today?' Yes/No
- Be sure to tick 'Required'
-
If you wish to add additional questions, for example using this form to ask for student response to a prompt in the lesson, click +Add new
-
Click 'Settings' (the three dots in the top right corner) and set as follows:
- Enable Only people in my organization can respond. This will ensure that students need to log in
- Record Name
- One response per person
- Set your start and end date and time to be limited to the class time
- Include a thank you message if you wish
- Add a graphic background for the theme if you wish
- Preview your form on mobile phone view to see what students will view
- Once you are finished creating questions, click Share
- Select 'Only people in my organisation can respond'
- Select the QR code option, then select Download to save a copy of the QR code to your device
- You may print out the QR code for use in your classroom, or you can add the QR code to your PowerPoint slide deck
- If you want to use the link rather than a QR code, you can shorten it via bit.ly.com
